The easiest and fastest way to see if you prequalify for CareCredit is to use this link. You will get the prequalification decision within seconds and, if you prequalify, you can apply right away, with confidence. If you are approved, you will either be approved for the CareCredit credit card or the CareCredit Rewards Mastercard.
You can also apply over the phone, toll free, at (800) 677-0718. If you choose to call, you can use our automated system 24/7 or you can apply with a live agent between 9:00am-9:00pm Monday-Friday, Eastern Time. Applicants must be at least 21 years of age to apply by phone (must be 18 or older to apply). Applicants by phone must have written copy of the credit card agreement before proceeding, which can be printed from carecredit.com or you can obtain a copy from your provider.
In addition, you can apply at healthcare provider locations that accept CareCredit. At this time, applications via a provider and by phone are only for the CareCredit credit card. Once you have activated your card, a Rewards Points summary is included on your monthly billing statement. You can also check your point balance online by logging in to access your account summary and manage your rewards.
Once you have earned enough points for a reward, redeeming is simple. You can manage or redeem points once you log in or register in the CareCredit mobile app. See Contact Us for information on whom to contact for questions.
You will automatically earn Reward Points when you use your account to make qualifying purchases. You earn 1 point for every $1 you spend, except for the following purchases, which do not qualify: cash advances, balance transfers, purchases of $200 or more in the CareCredit network and at select retail locations where promotional financing is assigned to such purchases, and finance charges and fees. From time to time, you may be offered additional incentives ("Bonus Points") for purchases made with your Account.
There is no limit to the number of points you can earn.
You will not earn points on purchases greater than or equal to $200 made in the CareCredit network where promotional financing is available. You will, however, earn 4 points per $1 on purchases under $200 in the CareCredit network through 12/31/26.

Your points will expire if they are not redeemed within 24 months. If your account has a New York mailing address, you will be able to redeem any points you have accumulated for up to 90 days after we send you notice that your account is closed or Rewards Program has been modified, cancelled, closed or terminated (subject to availability of points). However, points will be forfeited at account closure if the account was delinquent or if the account was closed for fraud or other misuse.
