NO PURCHASE OR OBLIGATION NECESSARY TO ENTER OR WIN

Eligibility: The CareCreditGold.com Contest sponsored by CareCredit/GE Money is open and offered only to legal residents of the 50 United States and the District of Columbia age 18 or older as of the date of entry who are employees of a CareCredit enrolled provider. By entering, you agree to these Official Rules and the decisions of the Sponsor, which are final and binding in all respects. Void where prohibited by law. Subject to all applicable federal, state, and local laws and regulations. Entrants from organizations which prohibit or restrict participation in promotions such as these are not eligible to participate. Entrants are responsible for confirming with their organization any policies or restrictions concerning participation in sweepstakes and similar programs. Sponsor reserves the right, at its sole discretion, to disqualify any entry if it concludes that the entrant has tampered with the entry process or the operation of the Sweepstakes or has acted in violation of the Official Rules or any applicable federal, state, provincial or local laws or regulations.

Promotional Period: Contest begins on January 1, 2009 at 12:00 a.m. Eastern Time (“ET”) and ends on December 31, 2009 at 11:59 p.m. (ET) (the “Promotional Period”).

Entry: During the Promotional Period, there are two (2) ways to enter. 1) Go to CareCreditGold.com, enter your email address and navigate to any CareCredit program and you will receive (1) entry into the Drawing for each CareCredit program viewed. Entries are restricted to each unique email entered, in addition to each initial viewing of a CareCredit program. Duplicate e-mails are allowable only for each primary program viewed. E-mails must be received by end of business on December 31, 2009 to be eligible for entry. Determination as to when an e-mail was posted is at the sole discretion of CareCredit. 2) To enter without submitting an e-mail, on a 3 x 5 card hand-print your name, business address, business work number and date of birth and mail to: CareCreditGold.com Contest/Vision, PO Box 1710, Costa Mesa, CA 92628-9923 must be postmarked by December 31, 2009 and received by January 7, 2010. Mechanical reproductions will not be accepted. Each online entry is subject to the rules stated above. All entries become the property of the Sponsor and will not be returned.

Drawing: Approximately ten (10) days following the end of the Promotional Period, potential winner(s) will be selected by Mindshare Creative by random drawing from all eligible entries received during the Promotional Period. All decisions of the Sponsor and the judges are final and binding. Odds of winning are based on the number of eligible entries received. The potential winner will be notified by email, phone and/or U.S. Mail within thirty (30) days after the drawing and asked to complete and return within ten (10) days of issuance an affidavit of eligibility and liability release, and unless prohibited by law, a publicity release allowing use of the entrant’s name and likeness for advertising and publicity. Noncompliance within this time period, noncompliance with these Official Rules, or the return of any prize notification as undeliverable, may result in disqualification and the selection of an alternate winner. Prize will be awarded in the name of the entrant whose name appears as associated with the potential winning entry, unless organization policy prohibits employees from receiving prizes in their own name, in which event the prize will be awarded in the name of the organization or as otherwise provided under the organization’s policy.

Prizes: [10] $100 American Express® Gift Cards. Prizes are nontransferable and must be accepted as awarded. No cash or other substitution may be made, except by Sponsor which reserves the right to award a prize of comparable or greater value, at its sole discretion. All federal, state and local taxes are the responsibility of the winner. Prizes are awarded “as is” with no warranty or guarantee, either express or implied by Sponsor. Source of the prize is not a sponsor of, or involved in, the sweepstakes.

Release: Sweepstakes entrants (i) release and hold harmless Sponsor (CareCredit) and their respective parents, affiliates, subsidiaries, legal advisors, advertising and promotional agencies, and web suppliers, and their respective officers, directors, employees, agents and representatives (collectively, the “Released Parties”) from any and all losses, damages, rights, claims, and actions of any kind in connection with or participation in the Sweepstakes or resulting from acceptance, possession, or use of any prize, including, without limitation, personal injuries, death or property damage, and claims based on publicity rights, defamation, or invasion of privacy; and (ii) consent to the use by Sponsor and its designees of their names, addresses, likeness and/or any statements for advertising and promotion purposes in any and all media now or hereafter known, without additional compensation, notification or permission, except where prohibited by law. Released Parties are not responsible for any late, lost, incomplete, illegible, stolen, undelivered, misdirected or postage-due entries or e-mails, or for any computer, technical, printing, typographical, human or other error, including, without limitation, errors by any of the equipment or programming associated with or utilized in the Sweepstakes or which may occur in the printing, the offering or announcing of prizes, administration of the Sweepstakes or the processing of entries of any kind relating to or in connection with this Sweepstakes. Sponsor reserves the right, in its sole discretion, to suspend or cancel this Sweepstakes at any time, for any reason including but not limited to tampering, unauthorized intervention, fraud or other technical problem, security, or other problems or activities. In the event of cancellation, Sponsor reserves the right to award the prize(s) in a random drawing from all eligible, non-suspect entries received prior to the time of cancellation.

Disputes: All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, or the rights and obligations of entrants and Sponsor in connection with the Sweepstakes, shall be governed by, and construed in accordance with, the laws of the State of New York.
