GENERAL RELEASE FOR USE OF INFORMATION OR LIKENESS IN CARECREDIT AND/OR GE MARKETING OR MEDIA ACTIVITY

By checking the consent box, having full authority to execute this Release on behalf of myself, I hereby grant permission to CareCredit, GE and/or anyone authorized by GE to use the Materials as specified below for any and all advertising, promotional, trade, and/or other lawful purposes, in any form of media now known or later developed. This particularly includes print (e.g., case studies, ads, articles, and books), electronic media (e.g., the World Wide Web portion of the Internet or the GE intranet, and all other media. The materials may be used worldwide, and will be without any payment to me, my company or any other party.

The permitted Materials are my individual name, voice, image and quotes.

I warrant and represent to CareCredit and/or GE that the information covered by this Release are original to me. I understand and agree that CareCredit and/or GE is (are) and shall be the exclusive owner of any and all right, title, and interest, including copyright, in any and all advertising, promotional, Web pages/screens, and other materials containing the foregoing permitted items, except solely as to pre-existing rights in any of the items released hereunder. CareCredit and/or GE may translate, modify and supplement such materials without notice to me or my permission.
